We, the undersigned, recommend the above named student to audition for the JDC & O. This student is currently a faithful member of our school/church vocal music performing organization. This student is aware of the fact the he/she must remain an active member in good standing of the school/church performing group throughout his/her JDC & O experience. She/he is a qualified student, and is now enrolled in the above named grade level of this school. In the event that this student is accepted for membership in this chorus, we will use our influence to see that he/she is properly prepared, and all whose signatures appear on this application, will adhere to the Rules and Regulations of the JDC & O.
